The YMCA is currently recruiting for a part-time Program Coordinator for our Healthy Living Y chronic disease programs.  The Coordinator will work with local  healthcare facilities to improve the health and well-being of our community.

Essential Functions

  • Perform data entry of program participant information
  • Work with Healthy Living Director to enhance relationships with public health, physician, and payer communities and other referral networks to enhance awareness of and referrals to the chronic disease programs at the Y (Diabetes Prevention, LIVESTRONG at the Y, Pedaling for Parkinson's, etc.).
  • Track and maintain data of referrals.
  • Work closely with the Healthy Living Director to enhance program sustainability.
  • Serve as a liaison, ambassador, and advocate for the Healthy Living Y and chronic disease programs within public health, physician and prayer communities.
  • Respond to inquiries from the general public and members of the public health, physician and payer communities about the chronic disease programs.



  • Bachelor's degree in a related field or equivalent experience required
  • Must have outstanding communication skills
  • Computer skills required
  • The ability to build strong relationships, inspire confidence, and work effectively with a wide variety of stakeholders, including physicians, public health officials, employers, health seekers, and senior Y leaders and lifestyle coaches
  • Exceptional organizational skills
  • Experience in a health care field preferred
